What are the requirements of a band press pack and what things do you usually find missing?

Required:
1. Black and White photo
2. Bio (one sheet only)
3. Complete contact info- website, email and phone
4. CD minus the wrapping
Missing?
1. iTunes import data (track listing, album title, etc)
2. Lyrics are useful for radio DJ's
3. Phone numbers- THE most forgotten detail and important for those last minute interview requests or show booking.
